The Care at Home Team provide outcome-focused, person-centred care and support tailored with the individual enabling them to enjoy an independent life in the comfort of their own home and around those they love the most.

Day Care Hubs and Support Services
All of the Support Hubs provide a safe and stimulating environment to socialise and participate as you require in a variety of activities. Support can be accommodated for all adults over 16 years. We support those with additional or specialist needs, older adults, all support tailored for your outcomes.
